End of Life Planning: Preparing for the Future

Virginia Healthcare Directive

This state provided form includes:

1. A Health Care Decision Maker or "Agent" which is also known as a Medical Power of Attorney.

2. Healthcare Preferences and Instructions

3. Organ Donation

It can be filled out online or printed and filled out manually.

National POLST Form

The POLST form is filled out with your physician, typically when a terminal diagnosis has been given. This form is signed physician orders that must be followed by any healthcare provider.

Virginia Durable Do Not Resusciate

A Durable Do Not Resuscitate form is signed by a physician and is a standing order to any healthcare provider. This website gives instructions and the printable form which must be signed by both the patient or their agent and a physician. A Durable Do Not Resuscitate (DDNR) signed by the patient can only be revoked by the patient.

Five Wishes Booklet

Five Wishes provides a simple language advanced directive book. It is available in 30 languages and in both print and digital format. It includes physical, emotional and spiritual wishes.
